Details: Note:  Please read the complete description below before applying for this job.  Complete DescriptionCome join us and be a part of Something Special Inside.  Our patients and our visitors have said it for years.  There's just something special...something different about Provena Mercy Medical Center.Responsible for coordinating patient placement and bed capacity issues for designated inpatient units during specific time intervals.  Utilitzes problem-solving and system process skills to facilitate prompt and appropriate placement of patients from various settings to inpatient beds.  Interacts with physicians, staff and customers in ways that ensure positive outcomes and promote customer satisfaction.Specific Responsibilities:Works closely with all admitting and transferring departments to coordinate daily patient placement plan for patients awaiting inpatient admission or in-house transfer; notifies departments concerning patient placement needs. Assists unit-based staff with prioritization and planning in order to optimize bed availability and patient care; works with Clinical Unit Leaders and staff to encourage completion of pending patient transfers and discharges and facilitates availability of open beds to admit new patients. Coordinates communication between patient care units, physicians, environmental services, admitting, and all other patient care departments involved in the admission of patients. Utilizes nursing knowledge to triage assignment of patients to appropriate target unit, based on patient needs and bed availability; prioritizes patient needs based on available resources; makes decisions regarding patient placement based on knowledge of medical necessity. Applies admission criteria to ensure proper placement of patients; recommends switching patients if necessary to ensure the most appropriate patient placement to provide optimal patient care. Acts as patient advocate when space is limited or not available; keeps patient informed of expected wait time; follows-up with units to facilitate room preparation. Partners with Staff Nurses to assure smooth transition for the patient being admitted. Collaborates closely with physicians to ensure patient placement to the most appropriate nursing unit based on patient's specific needs; takes physician phone calls. Collaborates with Nursing Administrative Supervisors and Central Staffing Office to ensure consistent patient placement around the clock which optimizes matching of resources with patient care needs; reviews initial bed availability and unit census; verifies availability of beds and possible impending discharges through phone calls and in-person consultation; communicates with Nursing Administrative Supervisors regarding issues that arise during shift to ensure smooth transition to next shift. Monitors and analyzes patient placement and capacity processes; creates reports; participates in quality audits; makes recommendations for admission system improvements based on data collected. Participates in Thruput committees.   Requirements:Associates degree in health related field required. Bachelor's degree in a health related field preferred; Three years of nursing experience required, with supervisory experience preferred.Microsoft Office products; Ability to Navigate Electronic Medical Records will be necessary.
